Gummy smile: how can you identify the cause and choose a treatment?

Showing gum tissue when smiling does not, by itself, determine which procedure is suitable. The first step is to understand what contributes to that display and which changes make sense for each patient.

Why does the cause matter?

A smile brings together teeth, gums, lips and facial proportions. The same concern can arise in different situations: gum tissue may cover part of the teeth, the upper lip may move excessively or the bone structure may play a role. More than one factor may be present.

For this reason, comparing your smile with an online photograph is not enough to choose a technique. Diagnosis aims to identify what is happening in your particular case.

What is assessed during the consultation?

The consultation considers your concerns, gum health, the relationship between teeth and gums, lip position and movement, and facial proportions. Photographs, clinical examination and additional tests when needed help guide discussion of the treatment plan.

Explain what bothers you most: the amount of visible gum tissue, the perceived size of your teeth or the overall smile. These questions help organize the conversation but do not replace an assessment.

Does gingivoplasty solve every case?

No. Gingivoplasty reshapes the gum contour. In certain cases, planning includes clinical crown lengthening and assessment of the relationship with the bone. In others, the cause of gingival display is not limited to the gums.

An approach suitable for one patient may not meet another patient’s goals. The proposal should make clear what the procedure can change and which factors will remain.

What should you ask before deciding?

What is the main cause of the gingival display? Why was this technique proposed? Are there alternatives? What are the limitations, risks and postoperative care requirements?

Follow-up is also important to discuss. A photograph taken soon after treatment does not, by itself, show how the result will behave over time. Expectations and possibilities need to be discussed before a decision.

What about recovery?

Recovery depends on the technique, the extent of the procedure and each patient’s circumstances. Hygiene, diet and follow-up instructions form part of an individual plan. Another patient’s care instructions should not be used as a prescription for your own case.
